Utilizing research in wheelchair and seating selection and configuration for children with injury/dysfunction of the spinal cord.

OBJECTIVE: To determine how research regarding medicare power wheelchairs and medical scooters and seating selection and configuration could be applied to the pediatric population with spinal cord injury (SCI) and dysfunction. METHODS: Extensive literature reviews were completed on (a) powered mobility options and age-appropriateness for training; (b) manual wheelchair configuration, pushrim biomechanics, and propulsion ergonomics; and (c) cushion selection and use of pressure-mapping devices. The findings in the literature review then were compared with clinical observations from Shriners Hospitals for Children, Philadelphia’s Seating and Wheelchair Clinic. RESULTS/DISCUSSION: A significant amount of research is published on propulsion ergonomics and pushrim biomechanics in adults with SCI. However, this literature review noted that there was limited research available on cushion selection and power mobility configuration. Many of the conclusions drawn from these publications are applied to pediatric patients and used during the decision-making process of wheelchair and cushion selection. However, it is critical to tailor the assessment to the pediatric population to meet their medical and functional needs. CONCLUSION: Although some of the findings can be correlated to the pediatric population, there is a lack of research on wheelchair and seating selection and configuration as it specifically relates to the young person with SCI and dysfunction. Future studies need to be completed.

Free: The Four Letter “F” Word

Now that I’ve got your attention, Let me explain.
When I’m talking about the “F” word, it’s got nothing to do with swearing, nor the word that gets kids giggling when they overhear it in conversation, or perhaps on the TV. But it’s effect on your home business opportunity, mlm or other online venture can be equally as alarming.
The word is “Free”.
Sure, there are obviously those that have made huge amounts of money using free stuff, but my personal experience has shown that my success is a direct comparison to the depth that I’ve dug into my wallet. Why is this?
Well, for starters, we say that we’re looking for a business opportunity. Whether its a turnkey home business for moms or a home business for baby boomers, doesn’t really matter.
Now I’ve been self-employed for the majority of my life, but I have never owned a business, online or offline, that has been free. Neither would I want to. It’s the investment - cold hard cash - that ensures you keep to the right path.
It’s the cash that you depart with that ensures that you treat your venture as a business, and not a hobby. It’s the bank manager standing over you, making you feel as if it’s his own personal money.
My experience tells me that its got a lot to do with how we value our money. I don’t know if you can follow what I’m getting at, but I have found that anything that I got for free never held my interest, so there was a fat chance of being successful.
I also think that the free methods, systems and tools we have available have contributed to the online marketers disease of JFPTP syndrome. And what is that? No doubt you’ll recognize it when I spell it out. Its the “Jump From Pillar To Post” Syndrome.
The incessant dabbling with any and everything that passes into our inbox, in the vain attempt to find the legitimate make money online ways that are going to fill our bank accounts and allow us to work at the kitchen table in our underwear. I think that’s something best left to your imagination.
Once you invest in something, there’s this little voice we have that makes us justify our spending, so we doggedly carry on as long as we possibly can before we even think of giving up. Many is the time that I’ve found a residual income home business and spent more than I intended to, but this seems to be one of the reasons that I stick like glue and get as much out of it as I possibly can.
There’s self-esteem at risk here, I don’t want anyone to think that I’ve flushed my money down the proverbial toilet. So I carry on until I’m spent, departing with my tail firmly tucked between my legs, but with my integrity and dignity intact.
I’ve personally found that the program is not as significant in your success, its the marketing system that drives the program. I guess what I’m saying essentially, is that the answer is in the marketing.
I know you’ve heard it all before, but it’s worth of a reminder. Do you honestly think that McDonald’s is the best hamburger you can get? I think not. There’s bound to be a much better hamburger produced by your local mom and pop hamburger joint. If they could only plug into a marketing system like McDonald’s use, they would have success spilling out of their every pore. It’s the systems that work, or to put it in one word, duplication.
When you next come across something that sounds to good to be true, promising to make you squillions in cash before you can blink, take another look. Look outside the box. If it belongs to the four letter word brigade - it’s free - that’s giving you a pretty good indication of what it’s worth.
Look for duplication. You will need to involve other people. Is this something that you can see other people getting value out of? Because that’s what it boils down to. Operate your own home business opportunity, mlm or whatever venture you are involved with keeping in mind that your fastest way to success is by helping others.
Sounds like a line out of a bad TV advertisement, and no doubt you’ve heard it all before. But I just thought I’d repeat it again, because it’s just so true, and maybe you just haven’t listened properly yet.

Medicare program; conditions for payment of power mobility devices, including power wheelchairs and power-operated vehicles. Final rule

This final rule conforms our regulations to section 302(a)(2)(E)(iv) of the Medicare Prescription Drug, Improvement, and Modernization Act of 2003 and medical scooters. This rule defines the term power mobility devices (PMDs) as power wheelchairs and handicap scooters vehicles (POVs or scooters). It sets forth revised conditions for Medicare payment of PMDs and defines who may prescribe PMDs. This rule also requires a face-to-face examination of the beneficiary by the physician or treating practitioner, a written prescription, and receipt of pertinent parts of the medical record by the supplier within 45 days after the face-to-face examination that the durable medical equipment suppliers maintain in their records and make available to CMS or its agents upon request. Finally, this rule discusses CMS’ policy on documentation that may be requested by CMS or its agents to support a Medicare claim for payment, as well as the elimination of the Certificate of Medical Necessity (CMN) for PMDs.

The last years the new trend in network marketing, especially for those working online, is what is called a funded proposal. The process of generating leads that you can try to convert them to distributors in your MLM opportunity by selling them usually an infoproduct in the front-end. That method can be very useful for those that don’t have the financial resources to advertise directly themselves and their multi-level marketing business for a long period of time.

Reliability and validity of the power-mobility community driving assessment

The Power-Mobility Community Driving Assessment (PCDA) is a performance-based measure designed to assess driving performance of individuals using power wheelchairs or handicap scooters in community environments. This article reports the results of pilot testing and an evaluation of the assessment’s reliability and validity. Pilot testing was conducted with a random selection of Canadian occupational therapists working in the area of mobility. Although the response rate was very low, feedback confirmed the utility of the measure and contributed to one substantive scoring revision. Reliability and validity testing was conducted with a sample of 34 drivers. Internal consistency results were positive. Interrater reliability was fair to high but limited by the lack of variability in the scores. Construct validity hypotheses were tested on the relationships between PCDA scores and vision, perception, cognition, and environmental accessibility. Results indicated no relationships between the PCDA and perceptual and cognitive function and only a weak trend for a relationship with environmental accessibility. Concurrent validity was established: PCDA scores were positively associated with the judgments of therapists familiar with the driving performance of participants. In summary, the PCDA has moderate to good reliability, and content and concurrent validity results were found. More research is needed, particularly on the underlying constructs of successful driving performance. At this point, rehabilitation professionals and their clients are urged to use this assessment to establish driving performance rather than relying on assessments of perception, cognition, or environmental accessibility to predetermine whether someone will receive power mobility. Clinicians may find this a useful tool to identify where clients are able to drive safely in community settings, to identify specific learning needs, and, through those, to promote independent living for drivers of power-mobility devices.

We are always taking in new information but the question is WHAT are we learning? As human beings we are always growing, but we need to ask ourselves “in what direction”? Are we choosing the direction that we want to go?
Many people are committed to their “stories” which really are excuses for not taking action. They live their life by them. I used to be one of the people committed to my stories/excuses. Believe me, it was learned behavior. I learned it from many different sources such as family, school, friends and work environments. For example, because of the influence of others at work, I always believed that it was best for me to stay home 4 work nights each week and only be out late on a work night once a week. As a result there are many things I have not done because of that limiting belief that it was “not good for me to go out too many times during the week”. One of the things I missed out on was learning Spanish, something I have wanted to do for years.
So why is it that we should be making conscious choices about which information we take in so that we are Continuously Learning?

1) Protect your future for you and your family…
We can protect ourselves by learning new skills. With these new skills we can protect ourselves from being laid off or downsized. For example, if you were to go to school once a week and learn something that makes you a more valuable commodity in the marketplace or learned a new skill in a different arena, you could be prepared for downsizing or layoff when it happens. We know in this changing world that there is no escape from change, it surrounds us daily and the pace of change is accelerating.
Are you sitting around waiting for change or are you directing the change that happens to you? We know that we do not get a promotion without a test. Have you ever applied for a job that you really wanted? Did you study, do research on the company and their visions and goals. Prepare yourself for that interview/test? If you really wanted the job, then I am sure you did. Learning something new, preparing for that business opportunity protects us and allows us to get out of our “comfort zone”. Nothing ever happens in the comfort zone, we stay right where we are, even though the world is changing around us. Do you want to get left behind?
2) Learning can change your life
I have just completed a learning event with Live Out Loud and Loral Langemeier in Lake Tahoe. Believe me, it was a stretch and really assisted me in getting outside of my comfort zone. My life has changed for the better because of that experience and I am preparing myself for the future!
The events that have happened in my life where I have had to get out of my comfort zone have all in some way, big or small, improved my life. There are still times where I want to wallow in my comfort zone. However, now I know the perils of staying there, so I do not stay there very long.
When I first started on my journey of building my home business, our dream was to be able to sponsor 10 children with “The Plan”. As we have learned and grown through the process our focus is now to sponsor 100 children. The learning has changed our attitude about life and money, which of course has resulted in changing our lives. The focus of our life has changed for the better.
3) Learning can keep you young
Learning assists us in adapting to change. Think of a time when you dreaded having to learn something new. Why were you dreading it? Was it because you are not in the habit of learning? Did you believe it would be too hard? Did you not see any benefit for the learning? Getting back into the habit of learning is just like starting out on a bicycle, it takes a lot more effort to get going than it does once you have attained the desired speed. When we are learning we are involved with life, we are out with our peers or a whole new group of people.
What are new information are you feeding yourself? I realized that the fiction books I was feeding myself were simply a form of escape. Very similar to the young boy we met on the streets of Le Ciba in Honduras. He was one of the kids who were abandoned to the streets as young as the age of two. What he did to escape the reality of his own life was to sniff gasoline. After meeting him I realized that I was doing the same thing by reading too many fiction books.
After that I started feeding myself with more positive material. If this is one of your escape mechanisms and you desire to change it, you may want to switch the type of books that you read. Biographies are a great way to switch from fiction because they are stories too.
Once we have begun to change our learning habits by making conscious decisions about the information we feed ourselves on, the next step is to teach what we have learned. Teaching others is the best way to learn it even better. One of my great desires is for my family and friends to learn how not to get left behind. That is the reason I choose to write these articles, because assisting you in your learning also assists me in understanding better myself.

Obstacle avoidance for power wheelchair using bayesian neural network.

In this paper we present a real-time obstacle avoidance algorithm using a Bayesian neural network for a laser based medicare power wheelchairs system. The raw laser data is modified to accommodate the wheelchair dimensions, allowing the free-space to be determined accurately in real-time. Data acquisition is performed to collect the patterns required for training the neural network. A Bayesian frame work is applied to determine the optimal neural network structure for the training data. This neural network is trained under the supervision of the Bayesian rule and handicap scooters the obstacle avoidance task is then implemented for the wheelchair system. Initial results suggest this approach provides an effective solution for autonomous tasks, suggesting Bayesian neural networks may be useful for wider assistive technology applications

Satisfaction with technological equipment in individuals with tetraplegia following spinal cord injury

OBJECTIVE: To measure the acquisition, use and satisfaction with high technology equipment by spinal cord injured tetraplegic subjects. DESIGN AND SETTING: A questionnaire was mailed to 102 tetraplegic subjects who were hospitalised in the rehabilitation center of Kerpape (Ploemeur, France) between 1998 and 2004, and 59 subjects responded. The questionnaire asked about the use of telephones, computers, wheelchairs and environmental controls at home. RESULTS: When a piece of equipment was acquired, it was very often used. Patient satisfaction with equipment was 79.3%. Home phones and mobile phones were often used with options such as hands-free devices (78 and 59% respectively). A total of 64.4% of subjects acquired a manual wheelchair and 61% a power wheelchair. The most commonly acquired options on the power wheelchairs were the powered recline (73,7%) and tilt (71,1%) systems. All options were used but all were more desired than acquired. A total of 27.1% of subjects desired a pushrim-activated handicap scooters, but only 15.3% had acquired one; 695% of subjects had a computer. Communication was the first use for the computer (82.5%); 49.2% of subjects had acquired an environmental control system, but 20% desired one. The first reason for lack of acquisition was financial difficulties but also accessibility and information problems. The factor that influenced the acquisition and need for equipment was the degree of spinal cord injury. No other factor reduced patient satisfaction with equipment. CONCLUSION: Patients were satisfied with the equipment they acquired. But their needs, especially wheelchair options and environmental control systems, were not satisfied.
